What are some common challenges faced by Remote Home Health RNs, and how can they be managed?

Remote Home Health RNs often face challenges such as coordinating care across multiple patients and providers, managing documentation efficiently, and ensuring clear communication with both patients and the healthcare team. Working remotely can also make it more difficult to assess patients' environments and needs without in-person visits. To address these challenges, RNs leverage telehealth technology, maintain organized digital records, and establish regular check-ins with both patients and colleagues. Strong self-discipline and proactive communication skills are essential for success in this role.

What is a Remote Home Health RN?

A Remote Home Health RN is a registered nurse who provides patient care, support, and health assessments to individuals in their homes through telehealth or virtual platforms, rather than in-person visits. They coordinate care plans, monitor patient progress, educate patients and families, and communicate with other healthcare professionals using technology. This role allows nurses to deliver high-quality healthcare while working remotely, helping patients manage chronic conditions, recover from illness, or navigate post-hospitalization care.

Job description

Mountain Pacific was established in Montana, in 1973. Mountain Pacific is a nonprofit organization that partners within our communities to provide solutions for better health. Through federal, state and commercial funding and partnerships, we bring national public health priorities to the local, state or regional level, leveraging our dynamic staff members' wide range of expertise to advance better health and health care services.
If you are passionate about improving health outcomes and have the skills and experience to contribute, we are committed to making your unique situation fit into our culture.
We have an opening for a:
RN - Clinical Field Reviewer (Remote in Montana)

The RN Clinical Field Reviewer is a remote position that requires in-home nursing assessments which require the candidate to reside in one of the following counties in Montana: Missoula or Ravalli.
We are a team of Registered Nurses working remotely across the state of Montana completing in-home nursing assessments of eligible members living in the areas where we live for the Medicaid CFCS/PCS (Community First Choice Services/Personal Care Services) program. We conduct person-centered in-home nursing assessments focused on a member's functional abilities and their needs for ADL and IADL assistance initially, annually, and as needed if a change in condition occurs for members of all ages. We have a shared goal of keeping members independent in their homes with the assistance of paid caregivers and informal support. We work closely with provider agencies across the state to ensure that services are being delivered to members as authorized following policies set forth by the State of Montana DPHHS. We empower members to become participants in their care through active listening and planning with their input, provide care coordination, practice cultural humility with each client, and utilize evidence-based practice as a foundation for our interventions.
Mountain Pacific is looking for a Registered Nurse (RN) to join our team who lives in Missoula or Ravalli County to cover their area of the state. It's important for the ideal candidate to have experience with home visits and passion for helping others while working independently in the field. You set your own schedule and are responsible for meeting your timelines.
This position requires travel by personal car to locations throughout Missoula and Ravalli Counties in Montana to complete in-home assessments, reliable home internet, and a current Montana Registered Nursing (RN) license. Experience with home health, hospice, or long-term care is preferred. The position is full-time, flexible schedule that works 30-40 hours per week and includes your full hourly wage for the time to travel and mileage reimbursement at the current federal rate.
At Mountain Pacific we offer a robust benefits package!
  • 401(k) with an automatic 3% employer contribution
  • Medical, Dental, Vision, Life, Flex Spending Account, Dependent Care Flex, and Health Saving Account
  • Paid Vacation, Sick, Holiday, Short-Term Disability and Long-Term Disability Leave
  • And much more!
The target compensation for this position is $32.00 - $33.00 an hour. The first review of candidates will be on August 7, 2026, and continually reviewed every 5 days after. You may apply electronically or by emailing your letter of interest to [email protected]. Learn more about the company by visiting our website at www.mpqhf.org
Mountain Pacific is an equal opportunity employer and utilizes Simpliverified. We welcome applications from all qualified individuals, regardless of age, race, color, religion, sex, national origin, marital status, disability, veteran status, sexual orientation, or any other characteristic protected by law. We are committed to providing a safe, productive, and professional work environment that promotes the dignity and well-being of every employee.
