• Conduct in-depth psychosocial assessments for patients, identifying their emotional, social, and financial needs to create tailored care plans.

• Collaborate with community organizations to ensure post-discharge support services are readily available for patients, facilitating a smooth transition back into everyday life.
• Deliver education and emotional support to both patients and their families about available treatment options and local resources.
• Engage in crisis intervention techniques to promptly address any urgent psychological needs that arise during the care continuum.
• Participate actively in hospital committees and contribute your insights on social work initiatives to promote best practices across the organization.
• Maintain meticulous documentation pertaining to patient interactions, treatment plans, and follow-up care to comply with organizational policies and legal requirements.

You will serve a diverse patient population primarily in a detoxification setting, where individuals are facing substance use challenges. This role requires a unique sensitivity to the complex emotional landscapes of these patients, helping them navigate through their recovery with personalized support and interventions aimed at improving their overall mental health.

• Master’s degree in Social Work from an accredited institution.
• A current, active state licensure as a Licensed Clinical Social Worker (LCSW).
• In-depth knowledge of mental health disorders, crisis intervention methods, and relevant medical terminology.

• At least two years of clinical experience in a healthcare or hospital setting, preferably in mental health or substance use environments.
• Familiarity with evidence-based treatment modalities and a track record of successful patient outcomes.
