• Conduct thorough clinical assessments to understand the mental health needs of patients and their families, crafting individualized treatment plans utilizing telehealth technology.
• Implement evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), considering the unique challenges presented in a virtual setting.
• Maintain detailed clinical documentation and ensure adherence to ethical and professional standards as outlined by the National Association of Social Workers (NASW).
• Collaborate with a variety of mental health professionals through virtual meetings to develop holistic treatment strategies, ensuring continuity of care.
• Educate patients and families about mental health conditions, treatment methodologies, and community resources, thus promoting understanding and acceptance.
• Participate in ongoing education and training opportunities to stay current with industry best practices, leveraging digital platforms for skill enhancement.

In this role, you will primarily work with patients and families in hospice care, addressing the multifaceted emotional and psychological challenges that arise during end-of-life situations. You will provide support not just for patients, but also for their loved ones, helping them navigate grief and loss while fostering a supportive environment that encourages open dialogue and expression of emotions.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid license to practice as a Licensed Master Social Worker - Clinical (LMSW-C) in the state of Michigan is necessary.

• A minimum of two years of clinical experience, particularly in providing psychotherapy services in a remote or virtual environment.
• Proficient in delivering solution-focused interventions tailored for hospice care and end-of-life issues.
• Strong communication skills for building rapport and trust with clients through virtual interactions.
• Experience utilizing electronic health records (EHR) systems for documentation and compliance.
