As you work from home, you will be closely collaborating with a multidisciplinary team, including medical professionals and support staff, all dedicated to optimizing patient outcomes and fostering a supportive work culture rooted in teamwork and open communication.

• Directly engage with clients who exhibit symptoms of major mental health issues and severe emotional disorders through both in-office visits and community outreach efforts.
• Develop and maintain a positive rapport with clients, instilling trust and demonstrating empathy as the cornerstone of your practice.
• Conduct and document comprehensive assessments, while also facilitating evidence-based screenings to tailor interventions to client needs.
• Create and implement individualized care plans that integrate various services, ensuring smooth communication and collaboration among clients, caregivers, and healthcare providers.
• Provide concrete support through individual, group, and family therapy, addressing specific client concerns and ensuring that treatment aligns with their needs.
• Monitor client progress rigorously, ensuring timely follow-ups, attendance at meetings, and adherence to discharge protocols.
• Participate in on-call coverage on a rotational basis during and after program hours to ensure client needs are consistently met.
• Contribute to continuous improvement by participating in committees aimed at enhancing the delivery and quality of services.

Our services primarily focus on individuals facing significant life challenges, including but not limited to those dealing with major mental illnesses or severe emotional struggles. As a Licensed Clinical Social Worker in our hospice center, you will engage with patients and families grappling with end-of-life issues, navigating grief, and managing complex emotional responses. Your work will be crucial in helping clients achieve a sense of dignity and psychological well-being during sensitive times.

• A Bachelor’s Degree in Social Work is required, with a current LCSW license in the state of Tennessee.
• A minimum of two years of professional experience in clinical social work, particularly in mental health settings.
• Ability to effectively communicate and collaborate with diverse client populations and multidisciplinary teams.
• Strong understanding of ethical guidelines and a commitment to maintaining professional standards while ensuring confidentiality and client welfare.

• Experience working in hospice or palliative care settings, particularly with end-of-life issues.
• Familiarity with integrated care models and experience in developing contextualized treatment plans.
• Previous involvement in crisis intervention and the ability to navigate emergency situations with calmness and professionalism.

We offer a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
