THE ROLE:

We're looking for an experienced Outreach Representative to help grow awareness of PHS services and expand access to care for children and families throughout the Milwaukee/ Madison/ Wisconsin market.

This is a relationship-driven role for someone who enjoys being out in the community, meeting healthcare professionals, developing new partnerships, and turning relationships into meaningful growth opportunities.

You'll serve as a trusted PHS partner to hospitals, clinics, physicians, case managers, discharge planners, and other healthcare professionals, helping them understand our services and making it easier to connect children and families with the care they need.

  • Build and maintain strong relationships with hospitals, clinics, healthcare professionals, and other referral sources.
  • Develop new referral opportunities and strengthen existing accounts to support referral and admission growth.
  • Maintain a consistent presence throughout the assigned market and build relationships with key decision-makers and referral partners.
  • Educate healthcare professionals on PHS services, capabilities, and the medically complex pediatric populations we support.
  • Conduct presentations, meetings, tours, and educational sessions for healthcare professionals and community partners.
  • Represent PHS at conferences, networking events, community events, and professional gatherings.
  • Identify market trends, referral barriers, competitor activity, and emerging opportunities within the territory.
  • Track referral activity, market data, and outreach efforts to understand what's working and where additional opportunity exists.
  • Partner with Marketing to develop and execute outreach campaigns, presentations, events, and market-specific growth strategies.
  • Collaborate closely with operational and account management teams to ensure strong communication and a positive referral partner experience.
  • Maintain consistent follow-up with referral sources and proactively address questions, concerns, or barriers.
  • Independently manage territory priorities, relationships, outreach activity, and follow-up.

REQUIREMENTS

Required:

  • Bachelor's degree in Business, Healthcare Administration, Marketing, or a related field, or an equivalent combination of education and relevant experience.
  • 3+ years of healthcare sales, business development, outreach, referral development, or account management experience.
  • Strong relationship-building skills with the confidence to initiate new conversations and develop partnerships over time.
  • Excellent communication, presentation, and public-speaking skills.
  • Self-motivated and comfortable independently managing a territory and daily schedule.
  • Strong organization and follow-through with the ability to manage multiple accounts and priorities.
  • Ability to use market insights and referral data to identify opportunities and adjust outreach strategies.
  • Valid driver's license, reliable transportation, and ability to travel regularly throughout the assigned territory.
  • Proficiency with Microsoft Outlook, Word, Excel, and PowerPoint.

Preferred:

  • 5+ years of healthcare outreach, sales, business development, or account management experience.
  • Experience within pediatric healthcare, home care, infusion, clinical nutrition, respiratory/DME, or another post-acute healthcare environment.
  • Existing experience developing relationships with hospitals, clinics, physicians, case managers, discharge planners, or other referral sources.
  • Demonstrated success achieving referral, account-growth, or business-development goals.
  • Experience with Brightree, CRM platforms, or similar healthcare technology.
