Role Summary To provide the best motivation and health coaching to our patients through consistent, appropriate, timely, effective, and proactive and professional communication. You will drive clinical outcomes and engagement by providing comprehensive patient activation. Your role involves utilizing motivational interviewing to encourage positive health changes and supporting patients in creating and tracking SMART goals. Your Mission As a Care Manager with a Behavioral Health focus, you will maintain patient progress, adherence to care plans, and close HEDIS/care gaps. We are looking for individuals who want to thrive and grow with us. This role requires a high degree of adaptability; you must be able to follow directions and pivot quickly as we implement frequent process changes to meet company and patient needs. Daily Key Performance Indicators (KPIs)

  • Total Outbound Calls
    • Objective: Make a minimum of 60 phone calls per day. High performers understand that hitting connection goals may require exceeding this minimum.
  • Total Expected Connections
    • Objective: Connect with at least 25 patients per day.
  • Quality Assurance Score
    • Objective: Maintain a QA score of 90% or above.
  • Patient Stories & Surveys
    • Objective: Obtain 2 patient stories per month and ensure annual satisfaction surveys are completed.

Mandatory Requirements:

  • Clinical: Active MA, LPN, or LPT license/certification with 1+ years of Behavioral Health experience.
  • HIPAA Compliant Environment: A dedicated, private, and confidential workspace is required. No family, friends, or pets allowed in the workspace during work hours.
  • Professionalism: Mandatory "Camera-On" policy for all meetings.
  • Technical: Dual-monitor setup and hardwired high-speed internet.
