Crystal Lake Families & Kids Services is actively seeking a compassionate and dedicated Licensed Clinical Professional Counselor (LCPC) to join our remote team. Your primary responsibilities will include:

• Conducting comprehensive patient intakes that involve diagnostic and functional evaluations, followed by formulating and recommending individual treatment plans.
• Providing brief, focused therapeutic interventions for clients requiring immediate mental health support.
• Collaborating with primary care providers to identify patients who may benefit from specialty mental health services rather than primary care settings.
• Engaging in follow-up communications with clients to monitor their progress and make necessary adjustments to treatment plans.
• Creating relapse prevention strategies for at-risk patients to help maintain stability in their mental health.
• Developing educational resources that empower clients with self-management skills, enabling them to actively participate in their treatment journeys.
• Maintaining accurate clinical records and documentation of patient progress and treatment outcomes.
• Consulting on behavioral health services, contributing to program evaluation, and presenting recommendations for effective outcome management.
• Supporting the compliance of clients with any medical treatments prescribed by primary care providers, offering practical coping strategies for challenges such as medication side effects.

To succeed in this role, candidates must possess the following qualifications:

• Strong foundational knowledge of mental health assessment, treatment planning, and various therapeutic methodologies, including family therapy and behavioral therapy.
• Thorough understanding of developmental psychopathology and the ability to provide effective interventions based on DSM-IV diagnoses.
• Excellent communication skills and the ability to forge collaborative relationships with patients, families, and healthcare professionals.
• Strong organizational skills and the ability to manage clinical records comprehensively.

While not required, candidates with bilingual proficiency in English and Spanish are highly preferred, as this skill will enhance our ability to serve a broader client base. Additionally, experience working within a telehealth framework or with Federally Qualified Health Centers (FQHC) is advantageous but not mandatory.

The salary for this position ranges from USD 87,000 – USD 105,000 annually, commensurate with experience.
