OPTIONS COUNSELOR-VDC PROGRAM - NORTH CAROLINA

Peninsula Agency on Aging (PAA) is currently seeking an individual to serve as the Options Counselor for the Veteranโ€™s Directed Care (VDC) Program to provide case management and service coordination for eligible clients in North Carolina and surrounding areas.

Peninsula Agency on Aging has been providing services that have enabled older adults to live independently since 1974. Our mission is to support the independence and quality of life for residents primarily 60 years or over, by advocating, arranging, or providing vital human services.

Key Responsibilities

As the Options Counselor for the VDC Program, you will be instrumental in advising veterans and/or their caregivers of the services and resources available to support aging in place, and utilize interview skills to screen and complete assessments, provide options counseling, and develop comprehensive plans of care/service plans for veterans in North Carolina and surrounding areas. This position requires significant travel utilizing a personal vehicle. Mileage is reimbursed at the IRS mileage reimbursement rate.

Essential Skills and Experience

We are looking for applicants with any combination of education and experience equivalent to a Bachelorโ€™s Degree from accredited college or university in social work, human services, or related field. Applicant needs to be experienced with working with seniors and/or disabled adults, case management, excellent interviewing, oral, and written communication skills, and effective use of computers, software, and mobile technology. Applicants must also have the ability to model our values, and be vaccinated for COVID.
