This position is primarily remote, providing the flexibility to work from your own space while contributing to the operations of our Hospice Center. As a Licensed Independent Clinical Social Worker, you will be responsible for:

• Conducting comprehensive psychosocial assessments to evaluate patients' mental health statuses, strengths, and needs.
• Providing evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to individual patients and families.
• Collaborating with our multidisciplinary team to formulate and implement effective treatment plans that honor patient goals and preferences.
• Facilitating group therapy sessions and psychoeducation workshops to engage patients and their families in the treatment process.
• Supporting patients in navigating mental health systems, advocating for their rights, and ensuring access to necessary resources.
• Managing discharge planning and ongoing care coordination to facilitate smooth transitions and optimize care continuity.

You will be working primarily with patients facing end-of-life challenges, including individuals dealing with terminal illnesses and their families. Your role is vital as you support these patients by addressing their emotional, social, and psychological needs, ultimately guiding them and their supporters through a transformative and critical stage of their lives.

• A Master's degree in Social Work from an accredited institution is required.
• Current and unrestricted state licensure as a Licensed Independent Clinical Social Worker (LICSW).
• Deep understanding of mental health disorders, various therapeutic approaches, and crisis intervention tactics.
• Demonstrated ability to work in a collaborative, multidisciplinary environment.

• Prior experience in hospice or palliative care settings is highly preferred.
• Experience developing and implementing treatment plans involving diverse patient populations.
• Skilled in therapeutic group facilitation and psychoeducational programming.
