• Conduct thorough assessments to identify the social, emotional, and environmental challenges faced by clients, utilizing standardized tools and techniques to gather comprehensive data.
• Design and implement individualized service plans that clearly articulate goals, interventions, and methods for monitoring progress, ensuring alignment with clients’ unique situations.
• Provide high-quality individual and family counseling that addresses a wide range of issues, such as trauma, substance use, and mental health disorders.
• Connect clients with appropriate community resources and services, effectively acting as an advocate to bridge gaps and support their needs.
• Facilitate crisis intervention, risk assessments, and safety planning sessions to help clients navigate acute situations with sensitivity and professionalism.
• Work collaboratively with various community agencies and organizations to coordinate services and enhance support networks for clients.
• Engage in proactive community outreach initiatives to raise awareness of mental health resources and promote the services offered by Hartford Mental Healthcare Center.
• Participate in regular case conferences and team meetings to share insights, update on client progress, and undergo professional development.

The clientele you will serve at Hartford Mental Healthcare Center includes diverse populations facing critical social challenges such as homelessness, domestic violence, substance abuse, and poverty. Your expertise will aid clients in navigating these complexities, with an emphasis on fostering resilience, recovery, and empowerment throughout their healing journey.

• A Master’s degree in Social Work from an accredited program.
• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania.
• Strong understanding of social and economic justice issues, along with familiarity with community resources available to address these concerns.
• Exceptional communication and interpersonal skills, with an ability to build rapport with clients from diverse backgrounds.
• Knowledge of trauma-informed care principles and evidence-based practices.
