Rate of Pay: $39.66 to $42.06 (depending on licensure)

OVERVIEW OF PRIMARY RESPONSIBILITES:
This position is designed to bring behavioral health services directly into the community, serving individuals who have been recently discharged from behavioral health hospitalizations, lack access to remote technologies, or face barriers to onsite engagement. The clinician will provide individual, family, and group therapy using Evidence-Based Practices, with an emphasis on initial engagement and retention in care. Services will be provided in clients' homes, shelters, and other accessible locations, often integrating telemedicine tools.
PRIMARY FUNCTIONS:
  • Provide clinical therapy services (individual, family, group) in community-based and home settings.
  • Engage and retain clients who are disconnected from traditional services, including those discharged from psychiatric hospitals.
  • Use telehealth technologies to support care delivery in the field.
  • Collaborate closely with peer advocates, case managers, medical providers, and other members of the interdisciplinary team.
  • Utilize Evidence-Based Practices to support individuals with behavioral health needs, including crisis intervention when necessary.
  • Maintain a clinical caseload and complete all documentation in a timely, accurate, and compliant manner.
  • Support the implementation of specialized programming (e.g., first episode psychosis treatment).
  • Participate in weekend rotation hours as scheduled.
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:
  • Attend regular clinical supervision and ongoing training sessions.
  • Support and participate in program development efforts, especially as related to expansion of mobile and home-based services.
  • Utilize EMR and mobile documentation tools effectively in the field.
  • Assist with quality improvement efforts and outcome tracking.
  • Work collaboratively across CCBHC sites in Middletown, Newburgh, and surrounding communities.
QUALIFICATIONS AND ATTRIBUTES:
  • Strong interpersonal engagement skills with the ability to build rapport in diverse and underserved populations.
  • Flexibility and adaptability to provide care in changing and non-traditional settings.
  • Ability to work independently while maintaining communication with supervisors and team members.
  • Commitment to trauma-informed, recovery-oriented, and person-centered care.
  • Comfortable using technology and telehealth tools in a mobile setting.
EDUCATION AND EXPERIENCE:
Required:
  • Licensed Master Social Worker (LMSW) or LP-MHC or MFT in New York State.
  • Master's degree in Social Work or Mental Health Counseling.
  • Experience with crisis intervention and working with diverse populations.
  • Valid NYS driver's license and reliable transportation for community-based work.
Preferred:
  • Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC) or Licensed Marriage and Family Therapist (LMFT).
  • Experience with substance use disorders; CASAC credential is a plus.
  • Experience working with individuals diagnosed with intellectual/developmental disabilities.
  • Prior work experience in mobile, home-based, or outreach-focused behavioral health programs.
