This role is remote, allowing flexibility while working from home. However, communication and collaboration with multidisciplinary teams will be essential as you will engage in virtual meetings, case discussions, and ongoing professional development. • Conduct comprehensive assessments of clients' needs via phone or virtual consultations to determine areas of concern and develop tailored treatment plans.
• Provide emotional and psychological support to clients facing crises, working calmly and empathetically to stabilize situations and alleviate distress.
• Facilitate referrals to appropriate services such as counseling, health services, and community resources, ensuring clients receive comprehensive care.
• Engage clients in beneficial activities that promote mental well-being, enhancing their coping mechanisms through structured support and skill-building approaches.
• Maintain accurate and timely records of client interactions and services provided, preparing detailed reports for supervisory review.
• Actively participate in ongoing training and professional development opportunities to stay current with the best practices in the field of social work.
• Collaborate with other health care providers and social service agencies, ensuring a holistic approach to each client's care.

The primary focus of this role will be on vulnerable populations experiencing crises or challenges associated with mental health, domestic situations, and substance abuse. You will work with individuals and families who need immediate support and planning to foster long-term well-being, especially within the hospice setting where sensitive and compassionate care is crucial.

• A valid LCSW license with a Master’s degree in social work, psychology, or a related field.
• Exceptional listening and communication skills, with the ability to provide undivided attention to clients and assess their needs accurately.
• Strong crisis intervention skills and the ability to handle sensitive situations with calm and empathy.
• Competency with technology and proficiency in standard software applications such as Microsoft Word, Excel, Outlook, and PowerPoint.
• Valid driver's license and willingness to travel occasionally for training or client meetings, as needed.

• Previous experience in a hospice or healthcare setting, especially working with clients dealing with grief, loss, and end-of-life issues.
• Experience in providing therapy and interventions related to trauma, substance abuse, or domestic violence.
• Familiarity with community resources and the ability to rapidly connect clients with these services.

This position offers a competitive salary ranging from USD 90,000 to 100,000 annually, complemented by monthly productivity bonuses.
