Key Responsibilities

• Conduct thorough clinical assessments and develop personalized treatment plans for hospice patients and their families, using solution-focused interventions.
• Deliver evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), while adapting techniques for remote delivery that cater to the unique challenges of end-of-life care.
• Maintain meticulous clinical records utilizing secure electronic health record (EHR) systems, ensuring compliance with regulatory requirements and best practices.
• Collaborate with a multidisciplinary team of healthcare professionals, including nurses, physicians, and mental health specialists, through virtual consultations to coordinate comprehensive care.
• Engage in regular case consultations and peer review meetings via video conferencing to share insights and enhance the quality of care provided.
• Educate patients and their families about mental health topics, treatment options, and community resources, utilizing digital tools to provide consistent support.
• Stay current with emerging research and therapeutic practices through virtual training, webinars, and online resources, integrating these insights into your remote practice.

Patient Population / Client Focus

Your work will primarily focus on hospice patients, including individuals with terminal illnesses and their families. You will be pivotal in providing counseling and emotional support during a challenging phase of life, addressing both the psychological and social aspects of end-of-life care. Your role will help navigate feelings of grief, anxiety, and uncertainty, ensuring that patients and their families receive the emotional support they need.

Qualifications

• A Master’s degree in Social Work from an accredited institution is mandatory.
• Possession of a valid LCSW license in the state of Tennessee is required.
• A minimum of 2 years of clinical experience in providing psychotherapy services with a focus on end-of-life care is essential.
• Proficient in solution-focused psychotherapy interventions, adaptable for remote applications.
• Strong communication and interpersonal skills, with the ability to build rapport and trust with clients through virtual engagement.
