Key Responsibilities- Conduct thorough clinical assessments and evaluations to understand patients' emotional and psychological needs effectively.
- Develop and implement individualized treatment plans that incorporate evidence-based therapeutic interventions tailored to each patient's circumstances.
- Provide individual and family counseling sessions that help patients and their families cope with the emotional complexities of end-of-life care.
- Maintain accurate, timely case documentation in our electronic health record system to ensure the highest quality of patient care.
- Collaborate closely with a multidisciplinary team, including physicians, nurses, and chaplains, to ensure comprehensive care delivery.
- Stay informed on the latest research and trends in hospice and palliative care to continually refine and enhance therapeutic practices.
This role focuses on providing support to patients in hospice care and their families. You will work with individuals facing terminal illnesses, helping them and their loved ones navigate emotional distress, grief, and anxiety. This population requires sensitive and specialized approaches, making your expertise invaluable in improving their quality of life during this critical time.
Qualifications- A master's degree in Social Work from an accredited institution.
- A valid LCSW license in the State of Texas.
- Experience in mental health counseling, especially within hospice or palliative contexts, is highly desirable.
- Demonstrated understanding of solution-focused therapy, and familiarity with grief counseling methodologies.
- Strong interpersonal and communication skills, with the ability to work collaboratively with diverse teams and communicate effectively with patients and families.
- Prior experience working as a social worker in hospice or similar healthcare settings.
- Proficiency in using telehealth platforms, electronic health records, and other digital communication tools.
- A background in crisis intervention and support during bereavement periods.
