Key Responsibilities

• Conduct thorough clinical assessments to understand the mental health needs of hospice patients and their families.
• Develop tailored treatment plans based on evidence-based practices to support emotional well-being through grief, loss, and adjustment processes.
• Utilize integrative therapeutic approaches, such as solution-focused therapy and cognitive behavioral techniques, to effectively engage clients.
• Document clinical interactions promptly and accurately, ensuring compliance with all regulatory requirements and best practices.
• Collaborate with a diverse healthcare team to create comprehensive care plans that respect patients' physical, emotional, and spiritual needs.
• Provide education and resources to patients and families about mental health support and coping mechanisms in the hospice context.
• Engage in ongoing professional development, remaining informed of advancements in mental health care.

Patient Population / Client Focus

In this role, you will primarily work with terminally ill patients and their families who may be experiencing profound emotional distress, anxiety, and grief. Your expertise will be critical in helping them navigate this challenging time, providing them with emotional support and facilitating conversations about care preferences and end-of-life issues. The approaches you take can significantly improve patient experiences, allowing them to feel understood, valued, and supported.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in Georgia.
• Strong understanding of hospice principles and end-of-life care.
• Proficiency in evidence-based intervention strategies and solution-oriented therapy approaches.
• Exceptional interpersonal, communication, and problem-solving abilities.

Preferred Experience

• A minimum of two years of clinical experience, preferably in a hospice or palliative care setting.
