Key Responsibilities
  • Conduct ongoing assessments to accurately gauge patient needs and tailor treatment plans accordingly.
  • Maintain clear and ongoing communication with supervisors, healthcare team members, family members, and other stakeholders to monitor and report on patient progress.
  • Create and submit tentative schedules for appointments, home visits, and other therapeutic engagements to your supervisors.
  • Coordinate in-home visits as required, including conducting interviews, chart reviews, and data analysis to provide comprehensive support to clients.
  • Implement and monitor the effectiveness of treatment plans, ensuring clients receive quality care aligned with their goals.
  • Participate actively in Interdisciplinary Team Meetings to discuss client progress and treatment modifications.
  • Prepare and maintain thorough monthly and quarterly progress notes for each patient case.
  • Organize training sessions to ensure staff and team members are adept in program plan implementation and documentation.
  • Provide follow-up support through interviews, home visits, and monitoring sessions to ensure ongoing client engagement and satisfaction.
  • Serve as a liaison between the agency and the public, enhancing communication with the mental health community regarding support and resource availability.
  • Assist in developing procedures and training materials aimed at enhancing program efficacy.
Patient Population / Client Focus

In this role, you will have the opportunity to work with a variety of client populations, including both adults and youth, presenting diverse mental health challenges such as anxiety, depression, trauma, substance misuse, and relationship issues. Your expertise and compassionate approach will be instrumental in helping individuals lead healthier lives through targeted therapeutic interventions.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
  • Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon.
  • A minimum of two years of professional counseling experience, ideally within a clinical setting.
  • Strong knowledge of legal and ethical standards of counseling practice.
  • Exceptional interpersonal communication skills, with the ability to connect with clients from various backgrounds.
