Job description
The Home Health Intake and Scheduling Coordinator is responsible for coordinating and scheduling home health services for patients. This includes managing patient referrals, verifying insurance coverage, and scheduling appointments with clinicians. The coordinator will work closely with patients, families, clinicians, and insurance companies to ensure that all necessary services are provided in a timely and efficient manner.

JOB SUMMARY
The Home Health Intake and Scheduling Coordinator is a professional employee, who assists in the coordination of the agencyโ€™s daily operations including referral intake and scheduling for our clients and clinicians. This is a resource person for referral sources and works closely with other teammates to ensure adequate coordination and transition to home health services. It is a highly interactive position that requires the highest level of professionalism, capability to drive performance within the team, great computer skills and exceptional communication skills.
RESPONSIBILITIES
Intake:
  • Responsible for overall coordination of agency referrals for Medicare and non-Medicare clients under the direct supervision of Clinical Director
  • Resource person for referral sources; works closely with teammates to ensure adequate coordination and transition occurs to home health services
  • Receive and process patient referrals for home health services from physicians, facilities and staff and enter referral in EMR system
  • Verify insurance coverage is valid for clients (Medicare or non-Medicare) and obtain necessary authorizations
  • Maintain accurate and up-to-date records of client information and services provided
  • Ensure compliance with all regulatory requirements and company policies and procedures
  • Scan referral paperwork into EMR timely to ensure that it is available to clinicians
  • Communicate benefit information with referral source as necessary
  • Clearly communicate with referral sources and sales team on referral admission statuses
Scheduling and Administrative
  • Coordinate and schedule home health services for clients
  • Help coordinate skilled staffing assignments to meet client schedule needs and facilitate quality care
  • Communicate with clients, family members, clinicians, insurance companies, case managers and prospects to ensure that all necessary services are provided
  • Work closely with Clinical Director and Clinical Manager to coordinate services for new and existing clients
  • Data entry and effective communication with clinicians regarding any changes in client schedule or care
  • Maintain excellent relationships with clients, teammates, and providers
  • Assist with coordinating and overseeing the daily operations of the home health care agency
  • Assist skilled staff on issues that may arise โ€“ maintaining knowledge of and adhering to company policy
  • Perform other duties as assigned
ON CALL DUTIES
  • Participate in the after-hours, weekend and holiday staff scheduling coverage rotation.
  • Available to receive calls on off hours on occasion for issues, emergencies or back up.

REQUIREMENTS
  • An Associate degree in Business, Healthcare Administration, LPN License or equivalent work experience preferred
  • Three (3) years of related experience: home health, home care, assisted living, healthcare or personnel management experience.
  • Prior experience with insurance authorizations procedures
  • Experience in healthcare scheduling or related field
  • Excellent communication and interpersonal skills
  • Strong organizational and time management skills
  • Ability to work independently and as part of a team
  • Proficient in Microsoft Office and other computer applications. Be able to use Excel and Word at the intermediate level
  • Must demonstrate strong customer service orientation
  • Ability to maintain confidentiality of client information
  • Be able to handle sensitive or difficult issues with confidence and in a calm manner
  • Position is remote Monday through Friday with a rotation of On-Call weeknights and weekends
