• Conduct and document thorough bio-psychosocial assessments targeted at identifying mental health, substance use, and medical needs within a primary care framework.
• Deliver a range of behavioral health services, including individual and group therapy, intake processing, diagnostic evaluations, treatment planning, and crisis intervention. You will regularly communicate with medical providers to escalate patient needs, ensuring optimal care.
• Develop and maintain strong relationships with local resources and community agencies, providing clients with access to essential support services such as medication assistance programs.
• Engage in Quality Improvement initiatives aimed at enhancing service delivery and data collection protocols within the organization.

You will primarily work with individuals, couples, and families grappling with various mental health challenges, including anxiety, depression, and substance use disorders.
