Choice Physical Therapy & Wellness, Inc. is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a remote capacity. This full-time position allows you to leverage your clinical skills to support patients' mental health and well-being while providing essential services in a community health clinic setting. With an annual salary range of USD 90,000 – 100,000, you will play a pivotal role in delivering high-quality behavioral health services to a diverse population.

This is a remote position where you will work independently while being part of a supportive community health team. Our clinic values flexibility and provides you with the resources to conduct client assessments, document progress, and collaborate effectively with care providers. As a remote worker, you will have the tools necessary to stay connected and engage with both clients and colleagues.

• Conduct comprehensive biopsychosocial assessments tailored to individuals’ mental health, substance use, and medical needs, ensuring the assessments are age and culturally appropriate.
• Provide a variety of behavioral health services, which include individual and group counseling, intake evaluations, diagnostic assessments, treatment planning, and crisis intervention.
• Maintain thorough electronic documentation of all evaluations, counseling sessions, and follow-up care, ensuring compliance with state and federal regulations.
• Act as a liaison for clients by providing social work support, connecting them to local resources, community agencies, and medication assistance programs when needed.
• Collaborate with Quality Improvement teams to enhance data collection processes and contribute insights to improve clinical practices.
• Continuously monitor and assess patient progress, adapting treatment plans as necessary to achieve optimal outcomes.

You will primarily serve individuals, couples, and families facing various challenges related to mental health and substance use. Our clientele includes patients from diverse backgrounds and age groups, leading to a rich opportunity for culturally sensitive practice. The community health clinic setting enables you to engage with individuals who have limited access to behavioral health resources, emphasizing the importance of your role in fostering their wellness journey.

• Master’s degree in Social Work from an accredited institution.
• Valid and active state Licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of three years of post-licensure experience providing clinical social work services.
• Strong knowledge of crisis intervention strategies, counseling techniques, and the ability to make independent clinical judgments.
• Experience in conducting mental health evaluations and designing treatment plans.

• At least two years of experience working in a mental health or clinical setting, ideally within a community health context.
• Skills in group facilitation and experience leading therapeutic groups.
• Familiarity with electronic health record (EHR) systems for documentation and patient tracking.
