The Chronic Disease Health Educator provides evidence-based education to the public to promote wellness and prevent disease in Sedgwick County. This position plans, implements, and provides training on chronic disease and diabetes self-management, tobacco/smoking prevention and education, and other topics. The Chronic Disease Health Educator serves on workgroups and committees as assigned and recruits participants, schedules classes, and tracks performance indicators of the program. The position will provide support during public health emergencies.

Coordinate and provide education about chronic disease, including evidence-based diabetes, chronic disease self-management, and tobacco/smoking prevention and education

  • Create presentations for different audiences, including medical providers and youth and adults in high-risk populations
  • Facilitate or co-facilitate training using evidence-based and credible source practice methods and curricula
  • Promote understanding of medical treatment guidelines

Collaborate and communicate with community partners and the public

  • Conduct, participate in, and support community-based outreach programs in a variety of venues, which may be after regular work hours, including weekends
  • Refer community members to local, state, and national chronic disease-related and tobacco prevention and education-related resources
  • Coordinate with partners on messaging for the community to develop and communicate health messages to medical professionals, community partners, and the public
  • Communicate about the program and market services regarding chronic disease and tobacco prevention and education to medical professionals, community partners, and the public

Evaluate the effectiveness of chronic disease education

  • Collect qualitative and quantitative data on community knowledge and needs and evaluate the effectiveness of the program
  • Incorporate the needs and feedback of clients within SCHD programs and different communities
  • Create and distribute survey questions and assist with focus groups
  • Evaluate and tailor communication materials and training to the community

Minimum Qualifications: Two years of experience in public health, school or community health education, communication, management, nursing, or a health-related field. Two years of experience presenting information to groups in a classroom or business environment. Two years of experience providing customer service to the public. Up to two years of experience may be substituted with education that provides applicable knowledge and skills. Per Sedgwick County policy, this is a driving level position that requires a valid US driver's license without restrictions and current proof of automobile insurance. One year of experience with Microsoft Office Suite. Must complete Federal Emergency Management Administration (FEMA) IS 100 and 700 courses within 30 days of employment. Must become a certified Chronic Disease Self-Management Leader within 6 months of first available training after hire. Must become a trainer through KU Tobacco Treatment Specialist Training within 6 months of first available training after hire. Must become a certified Diabetes Self-Management Leader within 6 months of first available training after hire.

Preferred Qualifications: Three years of health education experience such as in a health educator role or instructing staff or the public about disease prevention. Two years of work experience with data analysis or data management. One year of work experience in public health or a medical field. Bachelor's degree in public health, education, school or community health education, communications, social work, or other related field. Bilingual in English and Spanish or Vietnamese. Certified Health Education Specialist (CHES), Certified in Public Health (CPH).
