In the role of Licensed Independent Clinical Social Worker, your responsibilities will include but are not limited to:

• Delivering psychotherapy and mental health services to patients in the hospice setting, addressing conditions like anxiety, depression, and post-traumatic stress disorder (PTSD).
• Conducting thorough assessments of patients' mental health statuses, focusing on their emotional, behavioral, and social functioning to accurately identify their needs.
• Developing and implementing evidence-based treatment plans that are culturally competent and individualized, ensuring they align with patients’ unique backgrounds and desires.
• Collaborating closely with physicians, psychiatrists, nurses, and other healthcare professionals to ensure patients receive coordinated and holistic care.
• Monitoring and reviewing patient progress regularly, making adjustments to treatment plans as necessary based on patient feedback and responses.
• Providing education and resources to patients and their families, addressing mental health conditions, treatment options, and available community resources.
• Maintaining ethical standards of practice, including upholding confidentiality and securing informed consent for treatment.

This position primarily focuses on patients at the end of life, who may be experiencing a range of complex mental health challenges due to their health status. You will work to alleviate emotional distress, provide support during a critical time, and help families navigate the emotional landscape associated with hospice care. Your expertise will be crucial in enhancing their quality of life and ensuring comfort during their final stages.

To be considered for this position, candidates must meet the following qualifications:

• A Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Independent Clinical Social Worker (LICSW).
• A minimum of two to three years' experience providing psychotherapy and mental health services to patients facing significant health challenges.
• Demonstrated excellent communication and interpersonal skills, fostering positive relationships with patients, families, and colleagues.
• Robust clinical skills including the ability to perform comprehensive assessments and craft customized treatment plans.
• Experience utilizing electronic health records (EHRs) to manage patient information and streamline care processes.
• Adeptness in understanding various mental health conditions and an empathetic approach to patient care.
• Commitment to maintaining confidentiality and adhering to professional ethical standards.

While not required, candidates with prior experience in hospice care or palliative care settings will be given preference. Familiarity with grief counseling and end-of-life issues can also enhance the candidate's effectiveness in this role.
