This position is entirely remote, allowing you to engage with patients, colleagues, and teams virtually while providing flexible work hours. Our collaborative culture emphasizes teamwork, where you will work closely with a multidisciplinary rehabilitation team, including physical therapists, occupational therapists, and speech-language pathologists, to ensure holistic patient care. • Conduct thorough psychosocial assessments utilizing interviews, observations, and an in-depth review of medical and rehabilitation records to evaluate factors impacting recovery.
• Deliver individual and family counseling aimed at addressing emotional and behavioral challenges, assisting patients in coping with adjustments post-injury or illness.
• Work collaboratively with the rehabilitation team to create and implement individualized treatment plans that consider the unique psychosocial aspects of each patient.
• Facilitate connections for patients to access community resources, including vocational rehabilitation programs, support groups, and housing assistance, enhancing their recovery process.
• Educate patients and their families on available community resources and post-rehabilitation services that can aid in the transition back to daily life.
• Participate actively in discharge planning to ensure smooth transitions to home or alternative care settings, preparing patients for their next steps in recovery.

The patient population you will serve encompasses individuals recovering from serious medical events, including those facing emotional distress associated with adjusting to new life circumstances. Your role will focus on addressing the psychosocial dimensions of recovery, empowering patients by enhancing their emotional regulation and coping strategies. You will help patients develop community connections to provide a support system, thereby improving their quality of life and promoting successful reintegration into the community.

• A Master’s degree in Social Work from an accredited program is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is a prerequisite.
• Demonstrated understanding of mental health issues, adjustment challenges, grief, and effective coping mechanisms.
• Outstanding communication and interpersonal skills necessary for building rapport with patients and collaborating with multidisciplinary teams.

• Experience working in a rehabilitation or medical setting is preferred but not mandatory.
• A background in providing support to diverse patient populations is a plus.
