Key Responsibilities
  • Conduct comprehensive assessments of patients' mental health needs and create individualized treatment plans that align with their goals.
  • Maintain communication with supervisors, direct care staff, family members, and other stakeholders to provide updates on patients' progress.
  • Develop and manage a tentative schedule for patient appointments, home visits, and necessary observations, ensuring efficient utilization of time and resources.
  • Coordinate in-home visits for thorough evaluations, including chart reviews and staff interviews, to support ongoing patient care and documentation.
  • Ensure treatment plans are implemented effectively and monitor their impact on patient outcomes.

  • Document and complete monthly and quarterly progress notes that reflect patients' achievements and challenges.
  • Organize training for team members to certify their competency in implementing program plans and accurately maintaining documentation.
