• Conduct comprehensive mental health assessments while considering the medical conditions affecting patients' mental well-being.
• Design and implement integrated treatment plans that address both psychiatric and medical conditions, utilizing evidence-based practices.
• Facilitate individual and group therapy sessions tailored to the unique needs of each patient, ensuring a supportive and therapeutic environment.
• Actively collaborate with multidisciplinary medical providers to ensure coordinated and effective treatment outcomes.
• Provide crisis intervention support during mental health emergencies, applying your expertise to de-escalate situations and offer immediate care.
• Educate patients regarding the interplay of mental health and physical health, empowering them to take an active role in managing their conditions.
• Maintain meticulous and confidential documentation in compliance with legal and ethical standards, ensuring patient information is securely handled.

Your primary focus will be on individuals facing mental health challenges, particularly those with co-occurring medical conditions such as autoimmune disorders, substance use disorders, and chronic pain. You will be instrumental in helping these individuals navigate their complex needs, making a significant impact on their overall health and recovery journey.

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in the state of New York.
• Demonstrated experience in addressing the mental health needs of patients with co-occurring medical conditions.
• Exceptional interpersonal and communication skills, enabling effective engagement with patients and collaboration with healthcare providers.
• Proven ability to maintain professional boundaries and adhere to ethical guidelines while ensuring patient confidentiality.
• Proficiency with computer applications and electronic health record systems, supporting effective workflow management.
• A strong capacity to work as part of a collaborative interdisciplinary team.

• Previous experience in a telehealth setting is highly advantageous, given the remote nature of this position.
• Familiarity with integrated care models and an understanding of the complexities involved in treating patients with both mental and physical health concerns.
• Active involvement in community outreach or education initiatives aimed at improving mental health awareness and access.
