• Engage directly with clients experiencing symptoms of major mental illnesses and severe emotional disorders, employing various therapeutic techniques both in virtual settings and community outreach.
• Collaborate closely with a multidisciplinary team to establish shared goals, ensuring a focused approach to care that meets the diverse needs of clients.
• Utilize evidence-based practices to conduct thorough, developmentally appropriate screenings, and arrange for detailed assessments as necessary, tailoring interventions to the individual needs of each client.
• Design and implement integrated care plans, facilitate access to necessary services, and promote communication between clients, families, and other healthcare providers.
• Provide crisis intervention support when needed, navigating sensitive situations with care and professionalism, consistently ensuring client safety.
• Maintain an active caseload that includes conducting individual, group, and family therapy sessions while documenting client progress efficiently, adhering to the organization’s reporting protocols.
• Contribute to quality improvement initiatives by participating in committees aimed at enhancing agency service delivery.

Your work will primarily involve a client demographic that includes individuals from underprivileged backgrounds facing complexities in their mental health suited to hospice care. These clients often deal with life-limiting conditions, requiring not only therapeutic interventions but also emotional support, guidance, and resource management as they navigate their healthcare journeys.

• A Bachelor's degree in Social Work or a closely related field is required; candidates must hold a valid Licensed Clinical Social Worker (LCSW) certification.
• A minimum of two years of experience in clinical social work, particularly within hospice, palliative care, or similar settings.
• A commitment to working effectively within diverse teams while fostering an inclusive environment that respects various cultural backgrounds.

• Previous experience working in remote settings or with telehealth methodologies.
• Familiarity with integrated care models and community resource navigation.
• Experience in crisis intervention and case management within palliative care services.

The annual salary for this position ranges from USD 70,000 to 85,000, commensurate with experience.
