Job description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position. The Remote Certified Primary Care Nurse practices autonomously, utilizing an extended scope of practice defined by several levels of regulatory guidance. These include provincial legislation; BC College of Nurses and Midwives scope of practice, practice standards and code of ethics; Nurse and Nurse Practitioners of BC decision support tools; Island Health policies, procedures and guidelines; and lastly individual nurse competencies.

The Remote Certified Primary Care Nurse works both independently and as a member of the multidisciplinary health care team, providing direct patient care and participating in health promotion activities for groups and individuals throughout the remote community. The approach to care must encompass community and cultural values that are client focused, aiming to maintain or achieve maximum health. This requires a broad focus of health care activities which incorporates disease prevention, chronic disease management and acute care across the lifespan.

It includes, but is not limited to, emergency assessment; treatment and palliation of illness and injury; assessment of health status; planning, implementation and evaluation of interventions and coordination of health services. Travel may be a requirement of this position. Transportation arrangements must meet the operational requirements of Island Health in accordance with the service assignment and may require the use of a personal vehicle.

QUALIFICATIONS: Education, Training And Experience Remote nursing certification or eligible for Remote Nursing Certification to be obtained within one (1) year of hire. Registration with BC College of Nurses and Midwives as a practicing RN(C) registrant. Three (3) years' recent related experience; knowledge of available programs and services, their interrelationships, and their function in delivering care in the community; or an equivalent combination of education, training and experience.

Basic Life Support, Level 'C' certification, Advanced Cardiac Life Support, Trauma nursing core course or equivalent. Completion of STI certification preferred. Valid Driver's license Skills And Abilities Demonstrates ability to provide client-focused, trauma-informed and culturally safe care.

Demonstrates ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning) and provide nursing care through evidence-based guidelines. Demonstrates ability to instruct clients and others about topics essential to health care and well-being. Demonstrates ability to apply knowledge of care planning and care coordination, including knowledge of community programs and services.

Demonstrates knowledge of community health practice, chronic disease management, emergency, geriatric and palliative nursing practice within a client and care giver centered care approach. Demonstrates conflict resolution, critical thinking and problem-solving skills. Utilizes sound judgment, good observation and assessment skills, tact and empathy.

Intervene in crisis or difficult situations. Demonstrates effective written, electronic and verbal communication to facilitate inter-professional collaborative practice, health environments and a respectful workplace for client, staff, students and partners/affiliates. Demonstrates ability to manage time and resources effectively.

Demonstrates self-awareness, developing and implementing own learning goals for continuous learning and professional development. Informs, applies and integrates research and evidence-based best practice related to nursing and health-related disciplines and the humanities, arts and sciences disciplines into professional practice. Demonstrates ability to operate related equipment and technology including relevant computer applications.
