As a Licensed Professional Counselor at Big Leap Family Guidance Centers, your key responsibilities will include:

• Conducting thorough patient intakes, emphasizing diagnostic assessments and functional evaluations, and providing tailored recommendations to the primary care provider regarding each patient's treatment goals.
• Delivering brief, focused therapeutic interventions for patients requiring mental health services.
• Guiding primary care providers on patient placements, identifying those who can be effectively managed within the primary setting versus those requiring specialized care.
• Initiating and conducting follow-ups to assess patient progress, adapting treatment approaches as necessary.
• Developing and implementing relapse prevention plans that empower patients to maintain stable functioning.
• Collaborating with the primary care team to monitor patients' health services related to behavioral health care and facilitating connections with external treatment providers as authorized.
• Documenting all patient interactions and advancements accurately in treatment charts, ensuring seamless communication with healthcare providers regarding patients’ needs and progress.
• Providing consultation on behavioral health services and contributing to program evaluation and outcome assessment.
• Equipping patients with self-management skills, offering educational resources to enhance their engagement in their treatment journeys.
• Maintaining adherence to the Code of Ethical Conduct for Clinical Professionals and participating in additional duties as assigned.

Your role will primarily involve working with a diverse patient population that includes individuals facing a range of mental health challenges and psychosocial issues. The clinic serves clients from various backgrounds, ensuring a culturally sensitive approach to treatment. You will be instrumental in facilitating access to mental health services, providing essential support to clients navigating difficult life circumstances.

The ideal candidate for this position will have:

• A minimum of a Master’s Degree in Clinical Counseling.
• A current Licensed Professional Counselor (LPC) license.
• Demonstrated knowledge of mental health assessment, treatment planning, and psychotherapeutic techniques, such as developmental psychopathology, systems in family therapy, and DSM-IV diagnoses.
• Strong ability to work collaboratively in an interdisciplinary team environment.
• Excellent interpersonal skills to establish and maintain effective relationships with patients, families, and team members.
• Effective communication skills, both written and verbal.

Preferably, candidates will possess:

• Bi-lingual proficiency in English and Spanish to better serve our diverse client population.
• Experience with electronic health records (EHR) systems and telehealth platforms.
• Understanding of community resources that can be utilized to enhance patient support and care.

This position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
