bout the Role

We are seeking a Health Education Specialist / Diabetes Educator to support health promotion and wellness programs that help members adopt healthier lifestyles. This role provides health education, diabetes education and coaching, assessment, and behavior change counseling to improve self-management, medication adherence, lifestyle modification, and clinical outcomes for a diverse member population.

Preferred Certifications

  • Certified Diabetes Educator Specialist (CDE) or Certified Diabetes Care and Education Specialist (CDCES).
  • Certified Health Education Specialist (CHES), Michigan Licensed Registered Nurse (RN), or American College of Sports Medicine (ACSM) certification.
  • Certification or progress toward certification is highly preferred.

Required Qualifications

  • Bachelor’s degree in Health Education, Health Promotion, Nutrition, Nursing, Exercise Physiology, Social Work, or a related field.
  • Two years of experience in a health promotion, health education, or wellness setting.
  • Two years of experience delivering chronic condition coaching or diabetes education.
  • Clinical knowledge of disease prevention and wellness strategies.
  • Working knowledge of Microsoft Office Suite and strong administrative skills.

Key Responsibilities

  • Identify member health education needs through health assessments, online health risk appraisals, and telephonic outreach.
  • Provide telephonic outreach, counseling, and coaching on health prevention guidelines, diabetes condition management, and general wellness strategies.
  • Use motivational interviewing and behavior change techniques to help members set goals, develop action plans, and track progress.
  • Refer members to internal and external resources and collaborate with care partners to support a seamless continuum of care.
  • Document member interactions, assessments, care plans, and progress within required timeframes.
  • Research and communicate relevant topics in health promotion, wellness, and disease prevention.
  • Prepare education materials and deliver targeted wellness programs for designated populations.
  • Serve as a liaison between internal and external stakeholders to support prevention, wellness, and disease management programs.
  • Comply with applicable regulatory and licensing requirements, including NCQA, CMS, URAC, and HEDIS standards.
  • Monitor, evaluate, and report program effectiveness and participate in continuous improvement activities.
