Job description

As a Health Coach, you'll support patientsvirtually through ongoing coaching, education, and care coordination. You'll play a key role in driving engagement, behavior change, and overall patient experience - working closely with providers and the care team.

  • Conduct regular virtual check-ins to track progress and motivate patients
  • Support lifestyle, nutrition, and behavior change goals
  • Serve as a primary point of contact for patient questions and support
  • Coordinate with providers, dietitians, and care team members
  • Document patient interactions in the EHR
  • Escalate clinical concerns to licensed providers as needed

WhatWe'reLooking For
  • 1-5 years of experience in health coaching, patient navigation, care coordination, or a similar patient-facing role.
  • Interest or experience in obesity medicine, lifestyle change, chronic condition support.
  • Able to efficiently use and adapt to EHRs, telehealth platforms, and a variety of digital communication and workflow tools.
  • Strong organization skills and comfort managing multiple patient touchpoints.
