Position Overview

Gotham Enterprises Ltd is seeking a motivated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote team serving a Hospice Center. Key Responsibilities
  • Conduct detailed psychosocial assessments to evaluate clientsโ€™ emotional and psychological needs in hospice care.
  • Provide therapeutic interventions, including cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care, tailored to clients and their family dynamics.
  • Develop individualized care plans that encompass treatment goals, interventions, and appropriate resources while fostering collaboration with the healthcare team.
  • Facilitate individual, family, and group therapy sessions, ensuring that all discussions are respectful, supportive, and therapeutic.
  • Engage in crisis intervention, providing immediate support and safety planning for clients and families facing acute challenges.
  • Maintain accurate, timely, and confidential records in compliance with HIPAA and ethical standards, ensuring proper documentation of all client interactions.
  • Participate in team meetings and care plan reviews to discuss client progress, strategies, and support mechanisms with interdisciplinary team members.
Patient Population / Client Focus

This role primarily involves working with clients facing terminal illnesses and their families. You will support a diverse client population, including individuals with chronic health issues, terminal diagnoses, and psychological distress related to their conditions. Your work will impact their emotional well-being, assist families in coping with grief, and provide support throughout the dying process.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution is required.
  • Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Illinois.
  • Minimum of two years of clinical experience in mental health or outpatient settings, specifically within hospice or palliative care being a significant plus.
  • Demonstrated proficiency in evidence-based treatment modalities, crisis intervention, and supportive counseling techniques.
  • Excellent communication and interpersonal skills are crucial for effectively building rapport with clients and multidisciplinary team members.
Preferred Experience
  • Experience working within hospice, palliative care, or other healthcare settings focused on end-of-life services.
