• Act as a key liaison between clients, their families, and outpatient healthcare providers to ensure seamless coordination of care.
• Engage with insurance companies to manage pre-certifications and utilization reviews, enhancing client access to necessary services.
• Participate in comprehensive discharge and aftercare planning to support clients' continued recovery and adjustment post-treatment.
• Provide therapeutic meal support by modeling healthy relationships with food, promoting overall wellness and self-acceptance.
• Extend empathetic and affirming care tailored to the distinct circumstances and intersectional identities of each client.
• Maintain accurate documentation and uphold the standards set by regulatory and accrediting bodies within the healthcare field.

• A Master’s degree in Social Work from an accredited institution is required.
