Job description

POSITION SUMMARY
Responsible for activities related to the development, implementation and ongoing support of quality management and educational initiatives throughout assigned communities. This position focuses on communities in the Business Optimization and Business Assimilation Groups, and works in collaboration with those teams. This position assumes a primary role in ensuring that the communities onboarded to DSL (acquisition) and works with the optimization group to assist assigned communities with challenged areas.
This is a remote position open to candidates located near a major airport. Standard business hours are 8:30 a.m. to 5:30 p.m.; however, due to the travel-intensive nature of the role (50%-75% travel), work hours and availability are determined by customer and transaction needs.
This role is annually salary is $140k + 10% Annual Bonus.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
  • Assists with the implementation of Clinical policies and procedures, which reflect adherence to corporate and external regulatory guidelines. Reviews new regulatory directives and implements changes as needed in communities identified in the Optimization and Acquisition Groups.
  • Reviews and investigates resident complaints/Hotline Complaints as directed Ensures compliance with resident rights policies and works to resolve resident grievances.
  • Conducts compliance surveys at communities identified in the Assimilation and Optimization groups. Assist in coordinating actions plans to minimize liability and risks as a result of compliance surveys. Responsible for initial assessment of clinical regulatory compliance in newly acquired communities.
  • Participates in State and Federal surveys and compliance investigations as assigned.
  • Supports and follows up with communities in identifying trends and assists in the development of corrective action plans and troubleshooting resolutions. Reports findings of State and Federal surveys to the Divisional Operations Leaders.
  • Gathers performance improvements indicators and survey data to identify trends. Develops Plans of Corrective Actions with the Divisional Operations Leader (ROL), Executive Directors (ED) and Director of Health and Wellness (DHW).
  • Reviews prior surveys and Plans of Corrections for progress and compliance.
  • Schedules periodic site visits and remote support to provide supervision, clinical expertise, ensure communication, and to monitor the overall operation the clinical services department. Conducts facility reviews based on operational needs, changes in management and commitment to regulatory compliance.
  • Reviews all potential move-outs and at risk residents in the Optimization and Acquisition communities in collaboration with Region and Division Nurse. Reviews all potential move-in denials prior to any communication with prospective resident or family member
  • Provide in-service training and orientation to community staff regarding survey process, risk management, corporate compliance, performance improvement, etc.
  • Participates in the clinical review of residents as needed
  • Audits level of care assessments for accuracy and compliance in Optimization Communities.
  • Initiates education on Level of Care Assessments for Acquired Communities.
  • Participates in review of clinical risk scenarios at the community level to mitigate liability and risk
  • Works closely with the regional and community teams to review and interpret state regulations as needed
  • Participate in the interview and selection process for community DHWs as needed.
  • Orchestrates onboarding of new DHWs (Optimization) and assists with onboarding of Acquisition communities.
  • Participate in the clinical review of new move-ins as needed and in support of the DHW.
  • Maintains current knowledge of state regulations and monitor proposed changes to regulations
  • Develops into valued and trusted member of the Divisional team offering clinical expertise and guidance to RDO, RDS and HRM
  • Champion the company culture as all times.
  • Perform other duties as assigned

SUPERVISORY RESPONSIBILITIES:
  • Provides supervision and guidance to community administrators and Director of Health and Wellness staff and others as assigned to assure delivery of consistently high-quality care to residents.
  • Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws

QUALIFICATIONS:

Educational Requirements and Experience:
  • Current active nursing license required. Bachelor of Science or Master of Science in Nursing Preferred.
  • Maintains knowledge of regulatory compliance in all states of responsibility.
  • Experience in assisted living, home health or long-term care industries
  • Ten (10) years' nursing experience
  • Licensed Nurse in good standing
  • Five (5) year nursing management experience

Knowledge, Skills and Abilities:
  • Language Ability:
    • Ability to communicate effectively speaking the primary language of the residents
    • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
    • Ability to write routine reports and correspondence
  • Mathematical Skills:
    • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
  • Cognitive Demands:
    • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations
  • Computer Skills:
    • Proficiency in computer skills, Microsoft Office and ability to learn new applications
  • Competencies:
    • Must demonstrate an interest in working with a senior population

ENVIRONMENTAL ADAPTABILITY:
  • Frequent travel is required
  • Works primarily indoors in a climate controlled setting

  • Possible exposure to chemicals as identified in the SDS Manual

The employee must regularly lift and /or move up to 50 pounds independently and up to 100 pounds with assistance of a second employee and/or mechanical devices. Must be able to push up to 350 lb.
