Job description

OneCare Crisis Network is now hiring Mobile Crisis Management Crisis Support Specialists. The Mobile Crisis Management Crisis Support Specialist will provide crisis management services and shall demonstrate competencies in crisis response and crisis prevention.

Position Type: Contract/PRN, which means as needed

Duties:

Assesses individual client functional skills, behavioral health and symptoms in the community.

Develops individualized client crisis and discharge coordination plans:

Coordinates individuals, and families to treatment services

Services may include both behavioral health, medical and addictions treatment.

Performs care coordination duties.

Provides crisis assessment and intervention services in support of consumer including after-hours duties.

Provides coordination in support and management

Collaborates with other team members and community partners

Other duties as assigned by the Supervisor or Mobile Crisis Clinical Lead

Some community outreach and event attendance are required.

Minimum Requirements: A minimum of two year’s experience in providing crisis management services in the following settings: assertive outreach, assertive community treatment, emergency department or other service providing 24-hours-a-day, 7-days-a-week, response in emergent or urgent situations, attend Mobile Crisis Worker Certification Training Program offered by partner agency.

Proven strong client engagement skills.

Proven success collaborating with teams at various levels of care and with community partners.

Demonstrated capacity to individualize care and tailor services to individuals at varying stages of change.

Compassion towards individuals with Mental Health Crises, and co-occurring medical and mental health needs.

Experience working with diverse communities preferred.

Experience using EHR for documenting services provided

Must have the ability to get along with co-workers.

Ability to meet strict guideline with documentation process

Must have a valid driver’s license, and reliable transportation.

Skills and Knowledge: Requires a working knowledge of well-developed human relations, interviewing, and writing skills; along with interpersonal sensitivity and awareness of client situations and values. Working knowledge of community behavioral health and/or addictions concepts, linking and coordinating resources, treatment services, environment assessment, behavioral health disorders and community resources.
