As a part of our remote team, you will engage with patients through scheduled telehealth sessions. Our telehealth facility provides you with the technology to connect effectively with patients in a safe and secure online environment. This flexibility allows our counselors to implement care strategies that align with the specific needs of each patient from any location.

Your daily responsibilities will include:

• Conducting thorough assessments by reviewing patient health histories and preparing for initial evaluations.
• Developing and implementing individualized mental health care plans in accordance with established professional standards of practice.
• Interviewing patients and their families to identify educational needs, set treatment goals, and outline plans of care.
• Monitoring and evaluating the patient and family response to treatment, making necessary adjustments to care plans as required.
• Collaborating with interdisciplinary team members, including physicians, to coordinate the overall plan of care and effectively communicate patient progress.
• Making timely and appropriate referrals to other healthcare professionals when indicated.
• Upholding patient confidentiality in accordance with HIPAA regulations and maintaining accurate documentation in our medical record system.
• Conducting engaging workshops about mental wellness and healthy minds, ensuring that all participants benefit from the information shared.

You will primarily work with patients requiring mental health support related to chronic conditions, particularly cardiac patients. Understanding the unique challenges faced by this group allows you to provide targeted interventions and support that fosters both mental and physical health. Your role will involve educating patients and their families, guiding them through coping strategies that can lead to improved health outcomes.

To be considered for this position, candidates must hold a current license as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or be a licensed psychologist. A Bachelor’s degree in a mental health-related field is required. Candidates must demonstrate effective communication skills, both verbal and written, and possess the ability to establish rapport with clients.

Experience working in telehealth is highly preferred, as it requires specific skills in utilizing technology for patient care. Familiarity with managing a diverse patient demographic, along with a history of conducting educational workshops, will be beneficial in this role. The position offers a competitive salary ranging from $90,000 to $104,000 annually, dependent on client load and engagement levels. Our compensation package also includes a sign-on bonus, weekly direct deposit, and comprehensive benefits to support both your professional and personal well-being.
