This position allows for remote work, enabling you to connect with patients efficiently and effectively from your preferred location. Full-time employees work Monday through Friday from 9:00 AM to 5:00 PM, embracing a structured schedule that supports work-life balance.

• Conduct comprehensive assessments of patients’ mental health needs to understand their emotional, behavioral, and social challenges.
• Develop individualized treatment plans that incorporate evidence-based practices and cultural competence, ensuring our approach is tailored to the unique situations of each patient.
• Provide psychotherapy and other therapeutic interventions to patients experiencing conditions such as anxiety, depression, and post-traumatic stress disorder (PTSD), focusing on fostering resilience and coping strategies.
• Collaborate closely with an interdisciplinary team, including psychiatrists and primary care providers, to create a synchronized approach that enhances the quality of care delivered to patients.
• Monitor patient progress meticulously, making necessary adjustments to treatment plans based on ongoing assessments and feedback from both patients and healthcare collaborators.
• Educate and support patients and their families about mental health conditions, available treatment options, and community resources to empower them in their care journey.
• Uphold ethical and professional standards, ensuring confidentiality is maintained while obtaining informed consent for treatment.

This role is particularly focused on patients receiving hospice care—individuals who may be confronting life-limiting illnesses. Our approach emphasizes holistic care, addressing not only the mental health needs of patients but also providing support to their families during a transformative and often challenging time.

• Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of 2 years’ experience in providing psychotherapy and mental health services.
• Exceptional communication and interpersonal skills, enabling effective engagement with patients and collaboration with other healthcare professionals.
• Strong foundational clinical skills, particularly in assessment and evidence-based treatment planning.
• Familiarity with electronic health records (EHRs) for managing patient information and coordinating care efficiently.
• Deep understanding of mental health conditions and treatments relevant to hospice care, as well as awareness of the particular challenges faced by these patients.
• Demonstrate a steadfast commitment to maintaining confidentiality and adhering to best practices in ethics.

• Experience in hospice or palliative care settings is highly beneficial.
• Proficiency in developing community outreach programs that connect patients and families to additional resources.

The salary range for this position is competitive, with an annual compensation of $90,000 to $105,000, commensurate with experience and qualifications. Additionally, we offer a comprehensive benefits package including:
