About the Organization

At Carter Healthcare Inc., our mission is rooted in advocacy, education, and direct services aimed at changing lives within our community. We address pressing social issues by empowering individuals, families, and marginalized groups. Our organization stands out due to its integrated approach to social impact, combining research, policy advocacy, and extensive program development to tackle the root causes of social challenges. We specialize in a spectrum of services, including counseling, community initiatives, and holistic care management, to promote social justice and empower those we serve.

Work Environment

This position is fully remote, allowing you the flexibility to work from the comfort of your home while positively impacting the lives of hospice patients. Although the role primarily operates during standard business hours, occasional evening and weekend hours may be necessary for special events or urgent client needs. Your remote work setting does not compromise your connection with the team or the clients; regular virtual meetings and collaborative tools ensure seamless communication and teamwork.

Key Responsibilities
  • Conduct comprehensive assessments to evaluate the social, emotional, and psychological needs of patients and their families.
  • Provide individualized and group counseling, applying evidence-based therapeutic approaches tailored to the unique challenges faced by those in hospice care.
  • Develop personalized service plans that address the specific goals and objectives of clients, ensuring their needs are prioritized and met effectively.
  • Act as a passionate advocate for clients, ensuring their rights and access to resources are upheld, especially in navigating healthcare options and social services.
  • Engage in community program development, collaborative decision-making, and contribute to program evaluations and grant reporting initiatives.
  • Work alongside healthcare providers, community organizations, and volunteers to enhance service delivery and care coordination.
  • Participate actively in team meetings, training sessions, and community outreach activities to strengthen teamwork and elevate service effectiveness.
Patient Population / Client Focus

As a Licensed Clinical Social Worker, your primary focus will be on patients in hospice care, a sensitive and critical area that requires not only technical skill but also deep compassion and understanding. You will work with a diverse population, addressing intricate emotional and psychological needs associated with end-of-life care. Your role will involve supporting not just the patients but also their families, helping them navigate the complexities of grief, loss, and transition during this profound time.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • Comprehensive understanding of social justice issues, advocacy methods, and trauma-informed care practices.
  • Exceptional communication skills, complemented by a strong sense of empathy and the ability to build rapport with clients and their families.
  • Strong organizational skills, with the capacity to handle multiple cases and clients in a timely and efficient manner.
Preferred Experience
  • Previous experience working in a nonprofit organization or similar healthcare setting is highly valued.
  • Familiarity with community resources and support systems that can be leveraged for patient benefit.
  • Experience in providing emotional support and counseling to individuals facing life-altering circumstances.
  • Background in program development, evaluation, or grant reporting is a plus.
Compensation and Benefits

The annual salary for this role ranges between USD 75,000 and 100,000, complemented by a generous sign-on bonus. We recognize and value the contributions of our employees, offering a comprehensive health insurance package, which includes medical, dental, and vision coverage. Additional expectations include options for retirement plans with employer contributions, as well as a generous paid time-off policy that encompasses vacation days and personal time.

We encourage ongoing education and provide opportunities for further training and specialization in fields related to social work and hospice care. Our supportive environment nurtures career advancement pathways, ensuring that team members can actively seek growth within the organization and the nonprofit sector at large.
