• Conduct comprehensive assessments to evaluate the social, emotional, and psychological needs of patients and their families.
• Provide individualized and group counseling, applying evidence-based therapeutic approaches tailored to the unique challenges faced by those in hospice care.
• Develop personalized service plans that address the specific goals and objectives of clients, ensuring their needs are prioritized and met effectively.
• Act as a passionate advocate for clients, ensuring their rights and access to resources are upheld, especially in navigating healthcare options and social services.
• Engage in community program development, collaborative decision-making, and contribute to program evaluations and grant reporting initiatives.

The annual salary for this role ranges between USD 75,000 and 100,000, complemented by a generous sign-on bonus.
