Job description

Community Options, Inc. is a national non-profit agency providing residential group home, day habilitation and supported employment services to individuals with disabilities in 12 states.
We are seeking a Registered Nurse (RNN) in Hillsborough, NJ. The RN delivers care to individuals in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.
Responsibilities
  • Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
  • Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
  • Advise supervisor of all concerns and issues related to the provision of nursing services
  • Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations
  • Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
  • Train employees on the medical needs of assigned individuals
  • Monitor health and medical conditions of individuals and refer for medical services as needed
  • Monitor the use of medications through medication administration logs
  • Ensure health service supplies are available and replenished as needed
  • Ensure program documentation and billable records are completed accurately and timely
  • Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
  • Participate with individuals in community, social, and recreational activities
  • Participate in circles of support for personal futures planning
  • Follow strict OSHA procedures regarding blood borne pathogens
  • Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
  • Additional tasks and responsibilities may be assigned
Minimum Requirements
  • Bachelor’s Degree or equivalent experience in nursing field
  • Minimum of one year of experience providing nursing services to individuals with disabilities
  • Valid nursing license
  • Valid driver’s license with a satisfactory driving record
  • Complete all state and agency required training per state guidelines
  • Ability to calculate figures related to medication dosages
  • Understanding and commitment to community-based support for persons with disabilities
  • Ability to exercise good judgment and remain calm in crisis situations
  • Ability to establish a comfortable and supportive relationship with an individual receiving supports
  • Proficient with Microsoft Office
  • Excellent verbal and written communication skills
  • Ability to effectively present facts and recommendations
  • Ability to effectively communicate with program participants, third-party agencies, staff and members of the community
  • Excellent time management skills
Working Conditions
  • Work in office, residential programs, day programs, and community environments
  • May be exposed to changing weather conditions when working outside
  • Hours and weekly schedules may change due to business needs and may include evening and weekend hours
  • May be required to be on-call in cooperation with other management staff
  • Occasional overtime may be required
  • Frequent lifting, stretching, and other physical exertion may be required
  • May be required to lift or move 50+ pounds
