Job description

Seventh Haven Care Services is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to provide comprehensive clinical care to clients in a home care setting. This role includes conducting health assessments, assisting with care planning, and performing skilled nursing procedures to ensure each client’s medical needs are safely and effectively managed in their home.

  • Conduct initial and ongoing client health assessments
  • Assist in the development and implementation of individualized care plans
  • Provide skilled nursing services, including medication administration, wound care, and treatments as ordered
  • Monitor client health status and communicate concerns to the care team
  • Support and follow established care plans to ensure continuity of care
  • Maintain accurate and timely clinical documentation in compliance with state and agency standards
  • Educate clients and caregivers on health conditions, medications, and care techniques
  • Ensure compliance with Minnesota DHS regulations, as applicable
  • Collaborate with physicians, RNs, case managers, and other team members

Qualifications

For Registered Nurse (RN):

  • Current, active RN license in Minnesota
  • Associate or bachelor’s degree in nursing (2–4-year degree required)

For Licensed Practical Nurse (LPN):

  • Current, active LPN license in Minnesota
  • Completion of an accredited practical nursing program

For All Candidates:

  • Strong clinical and communication skills
  • Ability to work independently and as part of a team
  • Reliable transportation (if travel is required)

Preferred Qualifications

  • Experience with medication management
  • Experience with tracheostomy (trach) care
  • Experience with wound care management
  • Home care or community-based experience

Training will be provided for the right candidate.
