• Conduct thorough psychosocial assessments through direct interviews, detailed observations, and meticulous reviews of medical records. This will help identify emotional, social, and environmental factors that may affect patients’ recovery trajectories.
• Provide compassionate counseling and support to patients and their families, assisting them in navigating adjustment issues that may arise during the rehabilitation process.
• Collaborate with the rehabilitation team, including therapists and physicians, to formulate individualized treatment plans tailored to the psychosocial needs of each patient.
• Facilitate access to essential community resources such as support groups, vocational rehabilitation, and housing options, ensuring comprehensive support for patients beyond the rehabilitation center.
• Educate patients and their families regarding available community resources and post-rehabilitation services, promoting informed and empowered transitions towards independence.
• Engage actively in discharge planning, ensuring seamless transitions to home environments or other care settings, thus enhancing long-term recovery prospects.

As a Licensed Clinical Social Worker at Bridge Way Reflections Therapy, you will work with a diverse group of individuals overcoming significant challenges, including substance use disorders, emotional distress, and co-occurring mental health conditions. Your role is pivotal in addressing the psychosocial dimensions of these patients’ experiences, fostering resilience, and enabling their successful reintegration into the community.

• A Master's degree in Social Work from an accredited program.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of adjustment issues, grief processes, and effective coping mechanisms, essential for patient support.
• Exceptional communication and interpersonal skills, which are crucial for building rapport with patients and collaborating with healthcare professionals.

• Prior experience in a rehabilitation or medical setting, which would be advantageous in understanding the unique challenges faced by our patient population.
• Proficiency in using electronic health record systems and case management software to document patient progress and coordinate care effectively.
