Key Responsibilities
  • Conduct thorough psychosocial assessments by engaging with patients through interviews and observations, alongside the review of medical histories to identify emotional, social, and environmental factors influencing recovery.
  • Deliver counseling support to patients and their families, effectively addressing issues related to adjustment, grief, and emotional health.
  • Work closely with a multidisciplinary rehabilitation team to design tailored treatment plans that consider patientsโ€™ psychosocial needs.
  • Guide patients in navigating and accessing community resources such as support groups, vocational rehabilitation services, and housing assistance.
  • Facilitate transition processes by actively participating in discharge planning and follow-up care, ensuring seamless re-integration into home or alternate care facilities.
  • Educate patients and families on available community resources and post-rehabilitation services to promote long-term success.
Patient Population / Client Focus

You will be serving a diverse range of patients, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Kansas.
