• Conduct thorough psychosocial assessments by engaging with patients through interviews and observations, alongside the review of medical histories to identify emotional, social, and environmental factors influencing recovery.
• Deliver counseling support to patients and their families, effectively addressing issues related to adjustment, grief, and emotional health.
• Work closely with a multidisciplinary rehabilitation team to design tailored treatment plans that consider patients’ psychosocial needs.
• Guide patients in navigating and accessing community resources such as support groups, vocational rehabilitation services, and housing assistance.
• Facilitate transition processes by actively participating in discharge planning and follow-up care, ensuring seamless re-integration into home or alternate care facilities.
• Educate patients and families on available community resources and post-rehabilitation services to promote long-term success.

You will be serving a diverse range of patients, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Your expertise will be vital in addressing not only the psychological aspects of their rehabilitation but also the social and practical challenges they face. This comprehensive approach aims to maximize their independence and enhance their overall quality of life, making every interaction meaningful and impactful.

• A Master’s degree in Social Work from an accredited institution.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Kansas.
• Excellent communication and interpersonal skills, facilitating effective collaboration with patients, families, and interdisciplinary team members.

• Experience working in rehabilitation or medical settings, demonstrating an understanding of the unique challenges faced by patients in these environments.
• A strong foundation in counseling techniques and adjustment issues, particularly relating to grief and coping mechanisms.
