Position Summary

Under the supervision of the Buchanan C.A.R.E. Director, the C.A.R.E. Coordinator is part of an interagency team responsible for providing case management, mentorship, conflict mediation, and service coordination for enrolled students. This position supports the daily operation of the school site and collaborates closely with SFUSD-COE teachers, counselors, administrators, and community partners.

The C.A.R.E. Coordinator plays a key role in implementing individualized action plans that support student engagement, transition, and graduation, while ensuringaccurateprogram documentation andstrong communicationwith students, families, and partners.

Job Responsibilities

Program Operations & Administration

  • Lead scheduling and coordination of allexploratoriesand student intakes.
  • Maintainaccuratecommunicationregardingschedules, attendance, and reporting needs.
  • Enter, track, andmaintainstudent and program data as required by grant funders.
  • Assistwith opening and/or closing of the school site.
  • Support SFUSD lunch coordination and other on-site operational tasks.
  • Maintain inventories and contribute to a safe, clean, and accessible environment.
  • Support program finances, documentation, and general administrative needs.
  • Respond to phone calls, voicemails, and alumni communication.
  • Maintain working knowledge of YMCA programs, services, and schedules.
  • Attend staff meetings,trainings, andrequiredsafety courses.

Communication & Partnership Coordination

  • Develop andmaintainstrong communicationand positive working relationships with school staff, administrators, families, and partner agencies.
  • Maintain positive parent and guardian relationships; respond to concerns ina timelyand professional manner and notify the C.A.R.E. Director when needed.
  • Communicate student progress to parents/guardians when related to attendance or engagement.
  • Complete DCYF monthly reports and ensure accuracy of attendance documentation.
  • Support management of student databases and related reporting responsibilities.
  • Collaborate with partner sites and attend monthly multi-site wellness team meetings.
  • Represent the program professionally with external partners including SFUSDCOE, DCYF, JPD, SFPD, and other community organizations.

Student Support & Youth Development

  • Provide day-to-day student support by addressing questions, needs, and concerns.
  • Coach and mentor high school-aged youth, promoting social-emotional skill-building.
  • Support students in developing and achieving goals in their Individual Learning Plans.
  • Participate in case consultations, Child & Family Team meetings (CFTs), and IEPs asappropriate.
  • Facilitate life skills, advisory sessions, and enrichment workshops.
  • Co-lead field trips and activities onearly releasedays.

Safety, Conflict Mediation & Restorative Practices

  • Participate in the Buchanan Safety Committee.
  • Provide support to students requiring intensive emotional or behavioralassistance.
  • Assistteachers with behavior support and conflict mediation using restorative and de-escalation practices.
  • Facilitate restorative circles in partnership with Student Support staff.
  • Ensure adherence to YMCA and SFUSD policies for safety, supervision, mandated reporting, and risk management.
  • Model YMCA cause-driven values (Nurturing, Genuine, Determined, Hopeful, Welcoming) and Character Counts values of caring, respect, honesty, and responsibility.

Program Planning & Community Engagement

  • Contribute to the development and implementation of short- and long-term program plans for Buchanan Alternative Education Programs.
  • Coordinate family and community engagement events.
  • Support promotion of program offerings and community partnerships.

Work Environment & Physical Demands

  • Work is performed in a school campus environment, which may include classrooms, offices, outdoor areas, and community spaces.
  • Must be able to move throughout the campus, occasionallyliftand/or move up to 25 pounds, and perform tasks requiring visual, auditory, and physical capability, with or without reasonable accommodations.
