Position Summary:
The Client Care Coordinator works within Mindful Health to bridge communication among medical providers, outside professionals, and in-house behavioral health providers. They keep clients engaged in their care; provide same-day care for clients in need who originally come in for behavioral health appointments by reviewing skills, care plans, and/or connecting to resources. The Client Care Coordinator is responsible for guiding clients through their Mindful Health treatment journey by providing support, education, and assisting clients with questions or concerns.
Duties/Responsibilities:
  • Receives “warm hand-offs” of clients with identified behavioral health needs after completing a clinical intake assessment. Reviews recommended care plans, educates clients on treatment options, and connects to additional Mindful Health resources as appropriate.
  • As necessary, creates and maintains payment plans for clients participating in services.
  • Explain the recommended level of care to the client and provide information regarding cost, time, and commitment.
  • Acts as liaison between client and identified Mindful Health treatment team to problem solve any identified treatment barriers.
  • Act as a point of contact for communicating with referral sources and providing information as requested.
  • Provide excellent customer service by employing active listening techniques, displaying empathy and courtesy, and demonstrating effective communication through proper tone, pace, volume, grammar, and pronunciation.
  • Demonstrate sensitivity to individual client situations and show proactive behavior in addressing customer needs.
  • Manage client interactions to ensure client inquiries are handled completely and efficiently.
  • Escalate transactions when defined by protocols.
  • Demonstrate compliance with Mindful Health policies, including those relating to HIPAA compliance.
  • Perform other duties as assigned.

Education and Experience:
  • High school diploma required; Associate's degree preferred.
  • 1 + year(s) experience within a behavioral health facility/practice or health care setting, or equivalent
  • 1+ year(s)of experience with insurance experience preferred.
  • Familiarity with medical terminology is a plus.
  • Active CPR certification required
Physical Requirements:
  • Ability to alternate between sitting and standing for extended periods.
  • Ability to lift, hold, push, and pull 15 pounds.
  • Ability to operate a computer and/or laptop through proficient typing, clicking, and viewing a monitor for extended periods.
  • Ability to view computer monitors with close vision, color vision, depth perception, and the ability to adjust your focus with good hand-eye coordination.
  • Must be able to effectively communicate with clients/team members/customers, and clearly express oneself while exchanging information.
  • Ability to read, write, and comprehend through listening.
  • Ability to perform the essential job functions consistently, safely, and successfully with the ADA, FMLA, and other federal, state, and local standards, including meeting qualitative and/or quantitative productivity standards.
  • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA, and other federal, state, and local standards.
