Highlands Oncology began in 1996 with three physicians and a desire to change the face of oncology care in our community. Dr. Malcolm Hayward, Dr. Dan Bradford, and Dr. Thad Beck knew that patients do better at home, in the community where they live and work, and where their support system is already in place. Job Summary: The Center for Chest Care is a multidisciplinary lung clinic providing patients with access to Oncology, Pulmonology, and Cardiovascular Thoracic surgery in one visit. The Medical Assistan (Patient Navigator) role is to provide coordination and support to patients diagnosed with thoracic cancer or lung abnormality.
Job Duties / Responsibilities:
  • Schedule and coordinate a patient's care from the time of referral, and throughout the diagnosis and early treatment period.
  • Review incoming referrals and appropriately schedule work-up for New Patient visit.
  • New Patients must be contacted within 3 business days of receipt of referral and scheduled within 7 business days of being received.
  • Prepare patients chart for visit to Chest Clinic-obtain medical records, order diagnostics, compile patient information, and provide information to the patient's doctor prior to patient's clinic visit.
  • During clinic be able to present information to provider and to act as Navigator for patient.
  • May assist with rooming patients, obtaining vitals, room patients and enter required EMR documentation.
  • Promptly carrying out urgent referrals/orders. Urgent referrals should be sent on the same or next business day.
  • Carry out patient plans following the multidisciplinary conference.
  • Follow up on all outgoing orders to ensure external clinic receives orders. Ensure patients are scheduled for diagnostic testing in a timely manner.
  • Scan medical records on assigned patients.
  • Provide education and support to patients.
  • Return patient calls/Canopy tickets by 3PM on day of the incoming call.
  • Keep provider informed of patient progress, issues, or concerns.
  • Attend Multidisciplinary Chest conference each Tuesday at 5PM.
  • Enter assigned patient data on spreadsheet each month.
  • Maintain good working relationships with internal and external multidisciplinary staff
  • Must work well independently and as a team member
  • Maintains good communication with patients, referring providers, Chest Clinic doctors, co-workers and supervisors.
  • Other tasks assigned by supervisor.

Requirements
Required Skills/Abilities:
  • Must possess excellent communication skills both written and verbal.
  • Excellent people skills include, but not limited to adaptability, assertiveness, problem solving and teamwork.
  • Maintains a high standard of professional performance, behavior, and appearance.
  • Proficient in use of EMR (electronic medical record)
  • Ability to perform basic functions in Excel.
  • Typing skills

Certifications/Licenses/Education/or Experience in a related field:
  • Licensed Professional Nurse or Graduate from an accredited Medical Assistant Program or equivalent, or aptitude for the job duties and responsibilities.
  • Must have patient care experience.
  • At least two years of oncology, cardiovascular or pulmonary experience preferred.

Physical Requirements:
  • Sitting for prolonged periods of time at a desk.
  • Computer use that requires visual acuity, typing, use of mouse/keyboard, and staring at a screen for extended periods of time.
  • Occasionally lifting objects of up to 25 pounds such as files, office supplies, assisting patients when needed, etc.
  • Walking and standing for continuous periods around the office or clinic as needed.
  • Fine motor skills for tasks like typing, writing, handling small objects or equipment and materials.
