Position: Physical Therapist Assistant (PTA) – Home Health, PRN Schedule: PRN/Per Diem to start, with the ability to scale up to part-time or full-time based on your availability Location: North Salt Lake to bluffdale and east to west of slc, UT

Compensation: Paid per visit as a 1099 independent contractor

Contractor Requirements: This is a 1099 independent contractor position. Applicants must carry their own professional liability (malpractice) insurance, workers’ compensation waiver. An active contractor’s LLC is not required but is strongly preferred.

Position Summary ; We’re seeking a licensed Physical Therapist Assistant (PTA) to provide in-home physical therapy under the supervision of a licensed Physical Therapist. You’ll help homebound patients improve mobility, rebuild strength and independence, and enhance quality of life — with the flexibility of a contract, per-visit schedule.

Responsibilities

  • Deliver skilled PT interventions per the patient’s plan of care, under PT supervision
  • Educate patients and caregivers on home exercise programs and safety techniques
  • Track patient progress and communicate updates to the supervising PT
  • Complete timely, accurate clinical documentation (EMR/point-of-care charting)
  • Travel to patient homes within the designated areas of service

Qualifications

  • Active Utah state licensure as a Physical Therapist Assistant (PTA)
  • CPR/BLS certification (or able to obtain)
  • Valid driver’s license, reliable vehicle, and proof of auto insurance
  • Home health experience preferred, not required
  • Strong communication, time-management, and interpersonal skills
  • Comfortable managing an independent contractor caseload (own liability insurance, workers’ comp waiver, LLC preferred)
