The Role

The Physical Therapist Assistant works under the direction of a supervising licensed Physical Therapist to deliver individualized, evidence-informed physical therapy services within our Skilled Nursing and Subacute setting. This position is classified as Non-Exempt and reports to the Director of Rehabilitation, the supervising Physical Therapist, and the Facility Administrator.

Core responsibilities include:

  • Implementing resident treatment plans in alignment with the established plan of care
  • Delivering skilled therapeutic interventions targeting strength, mobility, balance, coordination, endurance, and functional performance
  • Monitoring, documenting, and reporting resident responses and any clinical changes to the supervising Physical Therapist
  • Educating residents and caregivers on home exercise programs and safety strategies as delegated
  • Completing accurate, timely, and compliant clinical documentation consistent with federal, state, payer, and organizational requirements
  • Maintaining adherence to HIPAA, infection control standards, OSHA regulations, patient rights, and all applicable California state practice standards
  • Supporting discharge planning and transition-of-care activities as directed
  • Participating in team meetings, care conferences, and quality assurance initiatives
  • Treating all residents with dignity, respect, and compassionate professionalism

Physical requirements include the ability to assist with patient transfers, mobility support, repositioning, prolonged standing and walking, bending, reaching, and lifting with or without assistive devices. The ability to assist in resident evacuation during emergency situations is also required.

What You Bring

  • Current, active, and unrestricted Physical Therapist Assistant license in the State of California
  • Graduate of an accredited Physical Therapist Assistant program — required
  • Successful completion of the NPTE for PTAs — required
  • Current CPR/BLS certification (or willingness to obtain per facility policy)
  • Excellent verbal and written communication skills
  • Composure and professionalism in dynamic, fast-paced clinical environments
  • Strong organizational skills and the ability to manage multiple priorities
  • A team-first mindset and a genuine passion for skilled nursing and subacute care
