Job Description

Completes independent home visits to veterans with complex medical and psychosocial conditions. Responsible for case management, assessment, triage, veteran education, resource coordination, medication and treatment coordination, and care planning. Collaborates with the HBPC interdisciplinary team which includes the physician preceptor, registered nurse, licensed practical nurse, occupational therapist/physical therapist, social work, dietitian, speech therapist, psychologist, pharmacist, and advanced practice provider. Collaborates with services internal and external to the VA to manage the care needs of the patients. Provides patient and family health education with a focus on self-management, prevention, health promotion, and wellness based on the patient's individual goals. Assists additional teams with home visits based on patient care needs. Demonstrates the ability to prioritize the sequence of assigned patients based on patients' needs. Utilizes HBPC procedures and policies to facilitate autonomy in providing care.
