Position Overview: As a Home Health PT, you'll work independently with patients to help them improve their mobility, strength, and overall physical function. You'll provide customized care plans and therapeutic interventions, helping individuals regain independence and enhance their quality of life—all from the comfort of their homes.

Key Responsibilities:

Evaluate and assess patients' physical conditions to develop tailored treatment plans

Implement and adjust physical therapy interventions focused on mobility, strength, balance, and function

Educate patients and caregivers on therapeutic exercises, techniques, and safety precautions

Collaborate with physicians and other healthcare professionals to coordinate patient care

Maintain accurate, timely documentation of patient progress and treatment plans

Participate in on-call rotation (if required)

Qualifications:

Licensed Physical Therapist (PT) in CA

Minimum of 1 year of clinical experience (home health preferred but not required)

Excellent communication, problem-solving, and organizational skills

Ability to work independently and manage a flexible schedule

Reliable transportation and valid driver's license

Knowledge of Medicare/Medicaid guidelines is a plus

EMR experience is preferred
