Role Summary

Are you a passionate advocate for older adults with a talent for building meaningful community connections? As an Outreach Ambassador, you are the inspiring face of our organization. In this pivotal role, you will drive program growth by developing strategic partnerships and educating the community on our premier model of care. You will act as a trusted guide for families, identifying and qualifying prospective patients to help them achieve their goal of living safely and independently in their own homes. Duties & Responsibilities
  • Strategic Growth: Develop and execute dynamic outreach plans to meet annual growth goals and expand our reach within the community.

  • Relationship Cultivation: Identify and nurture high-quality referral leads from healthcare providers, senior service networks, and community organizations.

  • Consultative Advocacy: Conduct heart-centered, consultative conversations with prospective patients and caregivers to align their unique needs with our comprehensive services.

  • Community Presence: Represent the organization with energy and professionalism at community events, educational presentations, and networking opportunities.

  • Team Collaboration: Partner seamlessly with the team, including enrollment staff and clinical assessors, to ensure a smooth and supportive transition for every new patient.

  • Data Management: Maintain accurate, timely documentation of all outreach and referral activities within the CRM to track success and guide future strategies.

  • Market Intelligence: Use your knowledge of local demographics and market trends to prioritize outreach efforts where they are needed most.

  • Continuous Improvement: Participate in professional development and team meetings to stay at the forefront of care principles and compliance standards.

Required Qualifications
  • Education: High school diploma or equivalent.

  • Experience: At least 3 years of experience in community outreach, relationship-based sales, or business development—preferably within healthcare or aging services.

  • Population Expertise: Minimum of 1 year of experience working with the frail and elderly population (training can be provided if this specific requirement is not yet met).

  • Technical Proficiency: Strong computer literacy, including Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and experience using CRM systems.

  • Communication: Exceptional public speaking, written, and verbal communication skills.

  • Compliance: Ability to pass a comprehensive background check and meet all medical clearance/immunization requirements for direct patient contact.

  • Mobility: Must have a valid driver’s license and reliable transportation for frequent local travel.
