Job description

This is a chance to help make a difference by opening your home to an individual with developmental and/or physical disabilities. Open your home to someone with a disability!

We are Now hiring someone in the Chattanooga, TN area. We are seeking someone who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support. This independently contracted position will provide a 24/7 family-like setting for an individual.

This role will help care for an individual in need of assistance with medical appointments and minor assistance with grooming. 

REQUIREMENTS: 

  • High School Diploma/GED
  • Valid Driver's License
  • Previous experience working as a Direct Support Professional is required
  • Will need to successfully complete trainings through Community Options
  • Must be open to supervisory visits in your home

Our Family Model Providers are contracted and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
