Green Acres Foundation is seeking an organized, self-motivated Seniors Support Worker who is flexible, has a positive attitude, and has superior customer service skills.If you are someone who values confidentiality, demonstrates initiative, and communicates with professionalism and tact, we encourage you to apply. You will be joining a collaborative and motivated team that values strong interpersonal relationships and a positive work environment.Reporting to the Director of Operations, the Seniors Support Worker is responsible for providing in-depth screenings to identify and connect seniors with appropriate social and health resources.Key responsibilities and duties: Assess client needsCreate and monitor plansCoordinate referralsFacilitate system navigationOvercome barriersSupport daily independenceCollaborate professionallyCommunity engagementGovernment reportingData managementQualifications and ExperienceBachelor of Social Work is considered an asset.Experience in the Human and Health Services field preferably with seniorsKnowledge of health systems for seniors, including resources to maintain independenceKnowledge of resources for seniors in rural communitiesRequired Skills:Current satisfactory Criminal Record CheckValid Class 5 driver's licenseCurrent satisfactory driver's abstractCurrent claims experience letterFirst Aid is considered an assetGreen Acres Foundation recognizes the diversity within the communities in which we serve and strives to ensure that our housing program and services, our facilities, and our employment opportunities are inclusive to all people.This personal information is being collected by Green Acres Foundation.
